Can You Use IPL on Tattoos, Moles, and Freckles? The Safety Guide Nobody Talks About
IPL works by targeting pigment. It's the entire mechanism: melanin in the hair shaft absorbs broad-spectrum light, converts it to heat, and thermally damages the follicle enough to interrupt the growth cycle. That's the science. But it raises an obvious question — what happens when the light hits other pigmented things on your skin?
Tattoos. Moles. Freckles. Birthmarks.
These aren't edge cases. A 2023 YouGov survey found that 26% of British adults have at least one tattoo, and the number climbs past 40% for under-40s. Moles are universal — the average adult has 10 to 40. Freckles are even more common. If you're using an IPL device at home, the odds that you'll need to navigate around pigmented skin features are high.

What Happens When IPL Light Hits Pigment
IPL devices emit broad-spectrum light — typically 400nm to 1200nm — with filters that narrow the output to wavelengths absorbed by melanin. When that light strikes a dark target, two things happen:
- Absorption: The pigment converts light energy to thermal energy
- Heating: The target structure heats rapidly — in hair removal, this means the follicle reaches temperatures that denature the dermal papilla cells responsible for regrowth
The critical variable is contrast. IPL works best when the hair is significantly darker than the surrounding skin, because the hair shaft absorbs the majority of the energy while the skin reflects or scatters most of it. This is why the Fitzpatrick scale matters so much for treatment safety — higher Fitzpatrick types have more epidermal melanin competing for the light, reducing the contrast ratio and increasing the risk of epidermal burns.
The same principle applies to non-hair pigmentation. Anything dark on your skin — tattoo ink, melanocytic nevi (moles), concentrated freckle clusters — will absorb IPL energy. The question is how much, and with what consequence.
Why You Must Never Use IPL Over Tattoos
Do not use IPL over tattooed skin. Full stop.
Tattoo ink is pigment designed to remain permanently in the dermis. When IPL light hits it, the ink particles absorb energy at dramatically higher rates than surrounding tissue. The result is rapid, concentrated heating — effectively a thermal event at the tattoo site.
What the research says
A 2017 case series published in the Journal of the American Academy of Dermatology documented IPL-induced tattoo complications including:
- Second-degree burns with blistering and subsequent scarring
- Ink darkening — certain pigments, particularly iron-oxide-based colours (reds, browns, flesh tones), can oxidise and darken permanently when exposed to high-intensity light
- Ink fragmentation and migration — thermal disruption can break ink particles apart and shift them through the dermis, distorting the tattoo
- Paradoxical darkening of cosmetic tattoos (eyebrow microblading, permanent eyeliner) is a well-documented phenomenon with IPL and laser exposure
The mechanism
A black tattoo absorbs across nearly the entire IPL spectrum. A coloured tattoo absorbs selectively — red ink absorbs green light strongly, blue ink absorbs red and near-infrared. But the net effect is the same: the ink becomes a heat sink, protecting the follicle beneath it while burning the skin above it. You won't achieve hair removal, and you will damage your skin and your tattoo.
Safe treatment distance
Keep the IPL treatment window at least 2 cm away from any tattoo edge. If you're treating an area with a small tattoo (e.g., a wrist design while treating forearms), cover the tattoo with a white medical tape or a damp white cloth before beginning treatment. White reflects most of the IPL spectrum rather than absorbing it, creating a safer barrier than dark fabric which would itself heat up.
For large tattoos covering significant portions of a treatment area (full sleeves, back pieces, chest plates), accept that IPL cannot treat that surface. The device cannot discriminate between hair pigment and ink pigment.
Moles: When to Treat Around Them (and When to Skip)
Moles — melanocytic naevi — are clusters of melanin-producing cells. They absorb IPL energy. The safety guidance is more nuanced than the tattoo rule, but the principle is straightforward.
The safe approach
Treat around moles. Do not flash directly over them.
Most moles sit at the junction of the epidermis and dermis (junctional naevi) or extend into the dermis (compound and intradermal naevi). When IPL light hits a mole, the concentrated melanin absorbs disproportionately, creating a small but real burn risk at the mole site. The mole itself may:
- Darken temporarily, then shed its surface layer
- Lighten permanently (unevenly — not a cosmetic outcome you want)
- Become irritated, crusted, or inflamed
- In rare cases, trigger changes that make a dermatologist's examination more difficult later
Covering technique
Use a white eyeliner pencil to draw over small moles before treatment. The white pigment reflects IPL light and prevents absorption. Alternatively, use small squares of white medical tape. Either method creates a physical and optical barrier.
After treatment, remove the covering and check the mole for any signs of irritation. If the mole shows redness, swelling, or crusting, discontinue treatment in that area and allow full healing — typically 5 to 7 days — before the next session.
When to skip entirely
If you have a treatment area with dense mole coverage — more than can be practically covered with a pencil — skip IPL on that area. The risk of accidental exposure multiplied across many moles isn't worth it. Areas like the upper back, where many people have scattered moles, require particularly careful mapping before treatment.
A note on atypical moles
If you have dysplastic naevi (atypical moles — larger, irregular borders, uneven colour), consult a dermatologist before using IPL anywhere near them. These moles already carry a slightly elevated risk of transformation, and you don't want thermal energy as a confounding variable. A dermatologist can clear the area for treatment or advise against it — and that advice is worth more than any internet guide.
Freckles and IPL: What Actually Happens
Freckles — ephelides — are patches of increased melanin production in the basal layer of the epidermis. Unlike moles, they don't involve a proliferation of melanocytes; they're simply areas where normal melanocytes are producing more pigment.
IPL will affect your freckles
When IPL light hits a freckle, the melanin absorbs energy just as it does in a hair shaft. The freckle will typically:
- Darken immediately — within hours to days of treatment, freckles often turn darker brown or nearly black. This is the melanin oxidising and the pigmented cells moving toward the surface
- Crust and flake off — over the following 7 to 14 days, the darkened freckle forms a micro-crust that sheds naturally
- Lighten or disappear — the underlying skin emerges with significantly less visible pigment
This is, in fact, the same mechanism that makes IPL an effective treatment for solar lentigines (sun spots). Many people actively seek IPL for freckle and pigmentation reduction. But the key word is actively — if you don't want your freckles to change, you need to know this will happen.
Freckles vs sun spots: the distinction matters

IPL treats both effectively, but the mechanism differs slightly. Sun spots involve a larger pigment target and often respond faster — sometimes after a single session. Freckles may require multiple sessions to fade significantly, and they'll return with UV exposure unless you're rigorous about SPF.
If you want to keep your freckles
If you value your freckles and don't want them altered, you have two options:
- Cover individual freckles with white eyeliner or tape, using the same technique described for moles
- Skip IPL on freckled areas entirely — for facial treatment especially, if you have dense facial freckling and want to preserve it, IPL may not be the right modality for you
There is no setting that targets hair while magically ignoring freckle melanin. The device cannot discriminate.
Birthmarks, Café-au-Lait Spots, and Other Pigmented Lesions
Café-au-lait macules, Becker's naevi, and congenital melanocytic naevi all contain concentrations of melanin. The same principle applies: IPL energy will be absorbed, and the lesion will heat up.
General rule: Do not treat over any pigmented birthmark without explicit dermatologist clearance. Some birthmarks contain deeper pigment that can produce unexpected and undesirable responses to light-based treatments. Café-au-lait spots, for instance, may lighten unevenly with IPL — producing a mottled appearance worse than the original uniform colour.
A Practical Pre-Treatment Checklist
Before every IPL session, do this:
- Map the treatment area under good light. Identify every tattoo, mole, freckle cluster, birthmark, and scar
- Cover tattoos with white medical tape or a damp white cloth. Verify coverage extends at least 2 cm beyond the tattoo edge in every direction
- Mark moles with a white eyeliner pencil. For moles larger than 5 mm, use white tape instead — a pencil line may not provide full coverage
- Decide about freckles — if you want to preserve them, cover them; if you're comfortable with them fading, proceed uncovered
- Check your Fitzpatrick type against the device's approved range. Darker skin types (Fitzpatrick V–VI) should not use most consumer IPL devices regardless of pigmented features — the baseline risk of epidermal burn is already elevated (see our Fitzpatrick scale guide)
- Do a patch test on a small area containing the pigmented features you plan to treat around. Wait 24 hours. Check for any unexpected reactions
What to Do If You Accidentally Flash Over a Tattoo or Mole
Accidents happen. If the treatment window catches the edge of a tattoo or you miss covering a mole:
Immediate steps
- Stop treatment on that area immediately
- Cool the site — apply a cold compress (not ice directly on skin) for 10 minutes. This reduces thermal spread in the tissue
- Apply a bland emollient — something with no active ingredients, no fragrance. Plain petroleum jelly or a basic barrier cream like Epaderm
- Do not apply: steroid creams (unless prescribed), numbing creams, ice directly, aloe vera with alcohol or fragrance, or any exfoliating product
Monitor for 48 hours
Watch for: - Blistering — fluid-filled bubbles at the site. Do not pop them. Keep the area clean and covered with a non-stick dressing - Intense redness or swelling that spreads beyond the flash site - Pain that worsens rather than improves over 24 hours
If any of these occur, seek medical attention — your GP or a walk-in clinic. Second-degree burns from IPL are treatable but shouldn't be managed at home.
For tattoo-specific incidents
If you flash over a tattoo and notice immediate darkening, blistering, or ink distortion, photograph it (for baseline documentation) and consult a dermatologist. Some tattoo pigments — particularly iron-oxide reds and browns — can undergo irreversible chemical changes when heated. Early intervention may limit the cosmetic damage.
Why This Matters Beyond Safety
There's a broader point here about how IPL works — one that connects to every other safety consideration in hair removal. IPL is not selective in the way a 755nm alexandrite laser is selective. A laser emits a single wavelength chosen for a specific chromophore target. IPL emits a band of wavelengths, and anything dark enough in the treatment field will absorb some of it.
That's why skin tone matters. Why sun exposure is contraindicated. And why tattoos, moles, and freckles demand attention — not because IPL is dangerous by design, but because its mechanism is pigment-agnostic.
This isn't a design flaw. It's the trade-off that makes IPL affordable, accessible, and effective for home use. But it comes with a responsibility: you have to understand what you're pointing the device at.
The Bottom Line
- Tattoos: Never flash over them. Cover with white tape, maintain 2 cm clearance. The burn risk is real and the ink damage can be permanent.
- Moles: Cover with white pencil or tape. Treat around them. If a mole shows any reaction after treatment, pause and let it heal.
- Freckles: IPL will likely lighten them. Decide whether you want that outcome before you start. If you value your freckles, cover them — the device cannot be set to ignore them.
- All other pigmented features: Default to avoidance. Get dermatologist clearance if you're uncertain.
IPL is safe when used with awareness. The people who get burned — literally — are the ones who assume the device knows what it's hitting. It doesn't. That's your job.
