But sensitive skin is not a contraindication for IPL. It is a signal to treat differently: lower energy, longer intervals, more deliberate aftercare. What "Sensitive Skin" Actually Means for IPL

Sensitive skin is not a clinical diagnosis — it is a symptom cluster. Dermatologists characterise it by: heightened neurosensory response (stinging, burning, itching), visible erythema (redness), and a compromised skin barrier that loses moisture faster than resilient skin types (Farage, 2019).

For IPL, this matters for two reasons:

  1. Absorption risk. A thinner, more permeable stratum corneum means light energy penetrates more efficiently — which sounds good, but actually translates to a narrower therapeutic window. The gap between "enough energy to damage the hair follicle" and "enough energy to damage surrounding tissue" is smaller.

  2. Inflammatory priming. Sensitive skin tends to have elevated baseline inflammation. Adding thermal energy from IPL pushes that further. Without cooling and aftercare, you are creating conditions for post-inflammatory hyperpigmentation (PIH) — dark spots that can take months to fade.

The Pre-Treatment Protocol: Patch Testing and Preparation

Before committing to a full session, run a patch test. This is non-negotiable for sensitive skin — skip it and you are treating a whole area blind.

How to Patch Test with an IPL Device

  1. Choose an inconspicuous spot. Inner forearm or a small patch on the treatment area. About the size of a £2 coin.

  2. Start at the lowest energy setting. Every IPL device has a power dial or intensity level. Begin at 1 (or the lowest available).

  3. Deliver 2-3 flashes in the patch area. Do not overlap.

  4. Wait 24 hours. Not 6 hours, not overnight — a full 24-hour cycle to observe the delayed response. Immediate redness is normal and expected; what you are watching for is the reaction at the 12- and 24-hour mark.

  5. Check for these signs: - Normal (green light): Mild pinkness that fades within 2-4 hours, no discomfort at 24 hours. - Caution (amber): Persistent redness beyond 8 hours, slight tenderness to touch. If this happens, wait 48 hours and re-test at the same setting — if it repeats, you have found your ceiling. - Stop (red): Blistering, crusting, darkening of the skin, pain lasting more than 24 hours. Do not proceed. This device at this setting is too aggressive for your skin.

Skin Preparation: The 72-Hour Lead-In

The three days before treatment matter. A compromised barrier going into IPL is a recipe for trouble.

  • Stop all actives. No retinoids, no AHAs/BHAs, no vitamin C serums, no benzoyl peroxide. These thin the stratum corneum and increase photosensitivity. Cease 72 hours before treatment.
  • No exfoliation. Physical scrubs, chemical peels, even vigorous cleansing — all increase surface sensitivity.
  • Hydrate the barrier. Use a bland, fragrance-free moisturiser with ceramides or squalane. The goal is barrier integrity, not active penetration.
  • No sun exposure on the treatment area. Sun-exposed skin has elevated melanin activity — more melanin means more light absorption in the epidermis, not the follicle. That equals more heat at the surface, more redness, and more PIH risk.
  • Shave, do not wax. For hair removal IPL, shave the area 12-24 hours before treatment. Waxing removes the hair shaft — the chromophore target — and irritates the follicle. Shaving leaves the root intact with minimal surface trauma.

IPL Device Settings for Sensitive Skin

You cannot treat sensitive skin the way you treat resilient skin. Here is how to adjust.

Start Low, Stay Low (At First)

The impulse is to crank up the power for faster results. Resist it. On sensitive skin, the goal is cumulative follicular damage across multiple sessions, not maximum energy in a single pass.

During Treatment: Real-Time Precautions

Avoid These Areas Completely

  • Active breakouts or open skin. Any disruption to the skin barrier — acne lesions, cuts, abrasions, eczema patches — is a direct channel for energy to the dermis without the stratum corneum's diffusion effect. Flash around them, not on them.

  • Areas with visible veins or telangiectasia. IPL can treat spider veins in the right hands, but at-home devices are not calibrated for vascular targets. Flash elsewhere.

Real-Time Feedback: What Is Normal

During the flash itself, you should feel a brief, warm snap — like a rubber band flick against the skin. This is the thermal pulse hitting the hair shaft. It should be noticeable but not painful.

After the flash, the skin may appear pink for 20-60 minutes. This is normal vasodilation. It should not:

  • Feel hot to the touch 30 minutes later
  • Show visible welts or hives
  • Produce blistering or weeping
  • Cause pain that lingers

If any of these occur, stop the session. Cool the area. Do not treat that area again at the same setting.

The Overlap Rule

The most common cause of IPL burns on sensitive skin is pulse overlap. When you place the treatment head down and it partially covers the spot you just flashed, that area receives double the intended energy. On resilient skin, that might mean extra redness for an hour. On sensitive skin, it can mean a linear burn.

Stamp deliberately. Lift fully. Place again with a small gap — the device head should touch skin that has not been treated in the last 3 seconds. Do not "paint" with overlapping strokes.

The first two days after IPL are when your skin barrier is most vulnerable.   • No hot showers, saunas, or steam rooms. Heat exacerbates post-IPL erythema and can push mild redness into prolonged inflammation.
  • No exercise that causes heavy sweating. Sweat contains salt, urea, and ammonia — all potential irritants on a compromised barrier. Wait 24-48 hours.
  • No makeup for 24 hours minimum. Even non-comedogenic formulas require removal — and removal means friction and cleansing. Give the skin a full day to reseal.
  • SPF 30+ every morning, reapplied. Post-IPL skin is photosensitive. UV exposure during the recovery window is the single biggest driver of PIH. Mineral sunscreens (zinc oxide, titanium dioxide) are generally better tolerated than chemical filters on reactive skin.

What to Expect

Some hair shedding (the "pepper spot" effect) may begin 5-14 days post-treatment. This is the treated follicles expelling dead hair shafts — it is a sign of efficacy, not irritation. Do not exfoliate or pluck them. Let them shed naturally.


Long-Term Aftercare: The Full Recovery Protocol

Between Sessions

Your skin needs to fully recover between IPL sessions. For sensitive skin, this means:

  • Minimum 7 days between sessions. Clinically, 10-14 days is safer if you experienced any lingering redness from the previous session.
  • Reintroduce actives gradually. Day 5+: reintroduce one active at a time, not your full routine at once. If your skin tolerated retinol before IPL, start at half frequency and build back up.

  • Any delayed reactions (days 2-5)

Stop the Session Immediately If:

  • You feel sharp pain (not a snap — a sting that makes you flinch)
  • The skin blanches white (this is a superficial burn)
  • Blisters form, even small ones
  • You smell burning hair that persists (brief singed-hair smell is normal; a burning-skin smell is not)

Seek Medical Advice If:

  • Blisters are larger than a 5p coin or are weeping
  • You develop a honey-coloured crust (sign of impetigo — a secondary bacterial infection)
  • Redness spreads beyond the treated area over the following 24-48 hours (cellulitis risk)
  • You develop systemic symptoms: fever, chills, or swollen lymph nodes near the treated area

These complications are rare with at-home IPL devices used at conservative settings, but sensitive skin has a narrower safety margin. Recognise them early.

Hyperpigmentation and Hypopigmentation

Dark spots (PIH) or light spots (hypopigmentation) that appear 2-4 weeks post-treatment are melanocyte responses to thermal injury. PIH is more common in Fitzpatrick III-V; hypopigmentation is rarer but more permanent.

If you notice pigment changes:

  1. Stop IPL on that area immediately.
  2. SPF 50+ daily — UV exposure darkens PIH.
